Create a New Form
Navigate to the Forms Section
Go to the Clients page.
Select the Forms tab.
Tap Create New Form.
Add Basic Form Details
Form Name – What you want to call your form.
Category – Choose Habit or Service:
Habit: Assign through habits to collect repeating information.
Service: Assign during a service to collect information at onboarding or assignment.
Description – Optional short summary of what the form is for.
Click Next.
Add Questions
Tap the + button in the bottom right corner.
Select your question type.
Enter your question and configure any additional settings based on the type.
Add as many questions as needed.
Click Next, review your form, then tap Create Form.
Your form will now appear in your Forms list.
Review & Create
After clicking Next, you’ll be taken to a review page.
Check the form name, category, description, and all questions to make sure everything is correct.
If you need to change anything, go back and edit.
When you're happy with the form, click Create Form.
Your form will now appear in your Forms list.
Edit a Form
Open the Forms tab.
Select the form you want to update.
Tap the three dots in the top right-hand corner.
Choose Edit.
Make your changes, then save.
Delete or Duplicate a Form
From the same three-dot menu, you can also choose to:
Duplicate — Create a copy of the form to edit separately.
Delete — Permanently remove the form.
